The remailing forms list all of the various services we provide and the associated pricing for each of those services. The forms below can be used for all remailing services where you are sending the actual items into our office for processing.  Please note that now we offer the ability for you to send us a PDF or DOC file electronically for us to print out, put in an envelope and mail for you.  If you are interested in our DIGITAL offerings, click HERE for more information.  Otherwise, print one of the forms below to include with the items that you mail to us for processing.  If you do not have a printer you can handwrite the information from the form onto a regular piece of paper and mail it to us.  As long as we understand what you need we can process it for you.

IMPORTANT:  Just because YOU can read YOUR handwriting does not mean we can as well unless you write it CLEARLY. We have had some cases where we had a very difficult time determining the full text of an address or the spelling of a recipient’s name.  Please PRINT CLEARLY and keep in mind that someone else has to read your handwriting.  Thank you.
